Q: I have tried several problems and about 40% of the time I get a wrong answer. Here is an example: pH 7.45, PaCO2 34, Na 134, Cl 85, HCO3 23. Your program ABGpro says that the primary disorder is respiratory alkalosis and if acute HCO3 should be 22.7 and if chronic expected is 21. If you do the calculations manually, the A.G. is 26 and the delta-delta is 39 which means that there should also be a metabolic acidosis and a metabolic alkalosis. Here is another example: pH 7.44, pCO2 12, Na 136, Cl 106, HCO3 8 Your program says primary disorder is respiratory alkalosis and if acute, expected HCO3 is 18.3 so concominant metabolic acidosis, if chronic then expect HCO3 is 10. If you do the calculations, the delta-delta is 18 and therefore there should be a non-anion gap acidosis in addition to the answer that was given. Why are the answers incorrect that the program is giving?
A: The calculations are correct. Delta-Delta should only be calculated when the Primary disorder is a metabolic acidosis.
